Complete enrollment is not available online at this time, but we encourage you to start the process using the forms below.

The Minnesota Department of Education (MDE) requires schools to have parents/guardians sign and/or provide various forms prior to a student starting school. Information required to complete the enrollment process:

General information

Address
Parent/guardian phone number of residence
Immunizations records for each child
Student records from previous school (if available)
Name, address, telephone and fax number of the last school attended
Family income information or food stamp (SNAP) case number (if filing an Application for Educational Benefits)

Document checklist

Fill out each form in its entirety. Forms must be signed and dated.

RWPS enrollment form
MDE demographic form
Release of records request (for students entering grades 1-12)
Minnesota language survey
Immunizations record or conscientious objection form
Transportation form
Certification of age. Options include:
- Copy of birth certificate
- Copy of immunization record (with child’s birthdate)
- Copy of a court decree (that shows child’s birthdate)
